East Hants Primary Care Network (PCN)

To meet the long-term health needs of our growing population, GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs). There are 6 surgeries within the East Hants PCN, The Grange, Forest/Badgerswood, Pinehill, Clanfield, Horndean and Rowlands Castle.

By having more health care professionals this means the PCN can bring more specialist skills and knowledge into the team. Our Team consists of Social Prescribers, Pharmacists, Dietician, Youth Mental Health and wellbeing coach, Health and Wellbeing Coach, Mental Health specialist.

The health of each individual patient is precious as well as their time so by ensuring that we are able to deliver the right care at the right time from the right qualified health care professional ensures that the patient is able to be seen quicker and GPs are there for when patients need to be seen for medical care rather than non-medical support.
